1. Two sets of verified articulated diagnostic casts mounted in centric relation (or adapted centric posture) on your articulator CLICK HERE  for help.
A diagnostic blueprint (wax-up) will be done with composite on the second set. Occlusal reduction guides, provisional stents and custom trays will also be fabricated on your casts. It will be ready to show your patient during your consultation appointment. 

2. Complete set of radiographs

3. Complete set of photographs  CLICK HERE for help.

4. Dental and periodontal charting.

5. TMJ/muscle/occlusal data on THIS FORM (This is a PDF file. If you don't have Adobe Acrobat Reader, download it free from http://www.adobe.com/products/acrobat/readstep2.html)

6. Patient concerns/exam and diagnosis summary (page 1), and patient classification along with pertinent technical information (page 2) filled out by you on THIS FORM. Dr. Tarantola will complete page 3 and 4 for you.
This can be used as a guide for your patient consultation. It will be in patient-friendly, understandable terms.
